首都醫(yī)科大學(xué)宣武醫(yī)院消化科由消化病房、消化科門診、消化內(nèi)鏡中心、胃腸功能檢查室組成，設(shè)有消化道早癌門診、結(jié)腸息肉門診、便秘門診、炎癥性腸病門診、膽胰疾病門診肝病門診等專病門診，病房床位44張。我科目前共有醫(yī)師21人，護(hù)士和技術(shù)人員44人，高級職稱7人，年門診量16萬余人次，每年收治住院病人2500余人次。在醫(yī)療、教學(xué)、科研等方面均具備雄厚的實(shí)力。

消化科是國家衛(wèi)計(jì)委專科及亞專科醫(yī)師培訓(xùn)基地，衛(wèi)生部臨床藥理基地，設(shè)有專業(yè)碩士、博士點(diǎn)。長期以來承擔(dān)了大量臨床教學(xué)工作，累計(jì)舉辦市級繼續(xù)教育項(xiàng)目10項(xiàng)，在臨床教學(xué)方面積累了豐富了經(jīng)驗(yàn)。自我科引進(jìn)中國第一臺內(nèi)鏡模擬訓(xùn)練機(jī)后，目前已建立以多臺內(nèi)鏡模擬訓(xùn)練機(jī)為核心的規(guī)范化消化內(nèi)鏡操作培訓(xùn)體系。科室還承擔(dān)首都醫(yī)科大學(xué)臨床教學(xué)任務(wù)，負(fù)責(zé)首都醫(yī)科大學(xué)博士研究生、碩士研究生、臨床醫(yī)療系本科生、國際學(xué)院留學(xué)生的教學(xué)工作。同時(shí)我科還承擔(dān)著包括國家自然基金在內(nèi)的多項(xiàng)科研課題。

為滿足廣大患者就診需要，消化內(nèi)科門診節(jié)假日全年無休，周末有專家門診開放。目前門診普通胃腸鏡檢查預(yù)約時(shí)間1周左右，無痛胃腸鏡檢查預(yù)約時(shí)間2周左右。同時(shí)我科開通24小時(shí)急診內(nèi)鏡綠色通道，建立了“急診-急診內(nèi)鏡-病房”一體化診療體系，診治了大量消化道出血、消化道異物及梗阻性黃疸的患者。

我科消化內(nèi)鏡中心擁有國內(nèi)最先進(jìn)的設(shè)備，可開展包括電子胃鏡、電子結(jié)腸鏡、電子小腸鏡、十二指腸鏡、電子超聲內(nèi)鏡、膠囊內(nèi)鏡等多項(xiàng)內(nèi)鏡檢查與治療技術(shù)，目前全年完成各種消化內(nèi)鏡的檢查超過20000例，各種內(nèi)鏡下治療超過1500例，包括各種消化道腫瘤(胃腸道早癌、粘膜下腫瘤等)的內(nèi)鏡下微創(chuàng)切除術(shù)及胰膽疾病的內(nèi)鏡下治療術(shù)，開展了EMR、ESD、IDUS、EUS-FNA、ERCP、MBM、金屬及塑料支架的置入、肝硬化食管胃底曲張靜脈出血的套扎硬化治療等多項(xiàng)技術(shù)。

1.  消化道早癌篩查及內(nèi)鏡下治療：食管癌、胃癌、大腸癌的早期診斷，早癌的內(nèi)鏡下治療（內(nèi)鏡下粘膜切除術(shù)(EMR)、內(nèi)鏡下粘膜剝離術(shù)(ESD)），進(jìn)展期消化道腫瘤的食道或腸道支架植入術(shù)。

2.消化系統(tǒng)危重癥診治：24小時(shí)急診內(nèi)鏡治療消化道大出血，急性重癥胰腺炎的診治。

3.腸道疾病診治：小腸疾病的診斷和治療、膠囊內(nèi)鏡、雙氣囊小腸鏡，腸道菌群移植治療。

4.超聲內(nèi)鏡：消化道粘膜下腫物、膽囊胰腺、縱隔腫物的超聲內(nèi)鏡檢查，及超聲內(nèi)鏡引導(dǎo)下細(xì)針穿刺術(shù)活檢(EUS-FNA)。

5.肝膽胰腺疾病組：經(jīng)內(nèi)鏡逆行性胰膽管造影(ERCP)、十二指腸乳頭切開取石術(shù)、膽總管內(nèi)引流術(shù)、胰管內(nèi)引流術(shù)、膽總管金屬或塑料支架植入術(shù)等。

6.胃腸動力疾病：便秘診斷和治療。賁門失遲緩癥的經(jīng)口內(nèi)鏡下肌切開術(shù)(POEM)。吞咽功能障礙的經(jīng)皮內(nèi)鏡下胃造瘺術(shù)。食道良性狹窄的內(nèi)鏡引導(dǎo)下的食管擴(kuò)張術(shù)。

Introduction of Gastroenterology Department:

The Gastroenterology Department of Xuanwu Hospital of Capital Medical University consists of a digestive ward, a digestive intensive care unit, a digestive clinic, a digestive endoscopy center and a gastrointestinal function examination room. It has a fatty liver clinic, a gastroesophageal reflux disease clinic, an ulcer disease clinic and parkinson disease constipation clinic, constipation clinic, follow-up clinic and other specialized clinics. There are 44 beds in the ward and 8 beds in the ICU. In recent years, under the leadership of the head of the department, Professor Zhang Mei, with the optimization of the professional structure as the core, a group of well-known experts and academic backbones with excellent skills have emerged, forming a reasonable talent echelon with a total of 21 physicians, nurses and technicians. There are 45 people, 95% of whom are masters and doctors and 6 have senior titles. It has strong comprehensive strength in medical, teaching, scientific research and other aspects.

Gastroenterology is the training base for specialist and sub-specialist physicians of the National Health and Family Planning Commission, and the clinical pharmacology base of the Ministry of Health, with professional master's and doctoral programs. It has undertaken a large number of clinical teaching work for a long time, has organized 10 municipal continuing education projects, and has accumulated rich experience in clinical teaching.

The Department of Gastroenterology gives full play to the resources and subject characteristics of the elderly and severe digestive system diseases, and has successively completed more than 30 major scientific research topics on elderly digestion. It led the establishment of the National Alliance for the Prevention and Control of Gastrointestinal Bleeding in the Elderly, established a data management platform for digestive diseases in the elderly, attracted more than 100 hospitals across the country, successfully held more than 10 high-level, large-scale alliance academic exchange activities, and more than 1,000 medical personnel. Greatly increased the academic influence of the department.

Gastroenterology ICU has become one of the largest gastroenterology ICUs in Beijing. Adhering to the development concept of “standardized diagnosis and treatment, precision medicine, and multidisciplinary cooperation”, it is good at gastrointestinal hemorrhage, acute severe pancreatitis, liver cirrhosis, hepatic encephalopathy, and sepsis. Diagnosis and treatment of critically ill patients with digestive disease and multiple organ failure are unique in the comprehensive assessment, diagnosis and treatment of critically ill digestive system in the elderly, realizing the greatest guarantee for the life safety of the elderly.

The digestive endoscopy center and gastrointestinal function examination room are equipped with various advanced equipment, which are good at endoscopic diagnosis and treatment and functional examination of digestive tract diseases. The 24-hour emergency endoscopy green channel has established an integrated diagnosis and treatment system of "Hospital Emergency-Emergency Endoscopy-Digestive ICU" to treat a large number of patients with gastrointestinal bleeding.
